Guidelines for the TPVES

Access to the Practical Evaluation Internship (TPVES)

For carrying out the Practical Evaluation Internship and obtaining the qualifying degree pursuant to Legislative Decree n.18 of 17 March 2020 it is necessary to fulfill the following obligations:

1. Having passed the sorting block for enrollment in the sixth year
2. Fill in  the request for carrying out the TPVES, and send the signed PDF to the teaching secretariat by inserting it in the Microsoft Forms form of the personal data sheet.
3. The assignments to the medical and surgical area departments will be made by the Teaching Secretariat, after the presentation of the request form and compatibly with the timing of the Graduation session to which you intend to apply.
4. Assignments to General Practitioners are made by the State Exams Office generally for the month preceding the graduation session to which one intends to apply.

Regulation of the TPVES

As required by current regulations, the practical-evaluation internship, aimed at obtaining the qualification to practice the profession of Doctor-Surgeon (pursuant to Legislative Decree No. 18 of 03/17/2020), to be carried out in the pre- -degree:

• is aimed at ascertaining the student’s skills relating to knowing how to do and knowing how to be a doctor, which consists in applying biomedical and clinical knowledge to medical practice, in solving questions of professional ethics and medical ethics, in demonstrating aptitude for solving clinical problems pertaining to the areas of medicine and surgery and related specialties, laboratory and instrumental diagnostics and public health, to apply the principles of effective communication;
• lasts a total of three months, is completed no earlier than the fifth year of the course and provided that all the fundamental exams relating to the first four years of the course have been successfully passed, as envisaged by the teaching regulations of the degree course, and is organized according to the provisions of the organization and the didactic regulation;
• it takes place for a number of hours corresponding to at least 5 credits for each month (each credit reserved for the internship must correspond to at least 20 hours of professionalizing teaching activity and no more than 5 hours of individual study) and is divided into the following periods, even if not consecutive : one month in the surgical area, one month in the medical area, one month in the specific area of ​​General Medicine, the latter to be carried out no earlier than the sixth year of the course, in the clinic of a General Medicine Doctor. The practical-assessment internship can also be carried out in periods in which frontal teaching activities are not scheduled (generally January, February, June, July, August, September) in order to reach the 100 certifiable hours.
• the certification of attendance and the evaluation of the traineeship periods take place under the direct responsibility and care of the university professor or the medical director, responsible for the structure attended by the trainee, and by the general practitioner, who issue formal attestation of attendance and express, after have evaluated the results relating to the skills demonstrated, if so, a judgment of suitability, on a booklet-diary, which is divided into a descriptive part of the activities carried out and an evaluation part of the skills demonstrated;
• it is considered passed only in case of achievement of the suitability judgment for each of the three periods.

SPECIFIC RULES

Each internship in the medical or surgical area can also be carried out in various clinical divisions of the medical or surgical area, within the OUs .  In this case, each tutor of the various divisions will issue a summary judgment on the candidate, from which the overall evaluation judgment of the candidate will derive, by the tutor coordinating the medical and/or surgical area; The internship as a general practitioner will take place with the doctors indicated by the professional Order. These doctors, identified by the Order(s) of Surgeons and Dentists of pertinence and assigned to the students by the State Exams Office, will express the overall evaluation judgment of the candidate.

IMPLEMENTING RULES

Attendance at the practical evaluation internship  begins upon presentation of a specific request, filled in by the student, to be delivered to the Teaching Secretariat of the Master’s Degree Course in Medicine and Surgery. The student must attach a copy of a valid identity document. The student will be assigned the medical and surgical area departments and the general practitioner where the student will have to attend, on the basis of the teaching organization of the fifth and sixth year of the course. The Teaching Secretariat will check the truthfulness of the declarations contained in the requests, in accordance with current legislation.

The student will be given, in sequence, three booklets, one for attendance in the medical area, one for attendance in the surgical area, one for attendance at the general practitioner’s surgery affiliated with the NHS. The carrying out of the practical-evaluative internship by the student must include 100 hours of attendance; these periods of attendance, compatibly with the didactic organization of the master’s degree course, must not necessarily coincide with the duration of a month; also the succession between the different periods, for each trainee, will be consequent to the didactic organization of the Study Course and will have to allow diversified successions in the different areas, in order to allow the correct execution of the activities,

The days and times of attendance will be noted in the booklet, describing the activities carried out also with reflections on what has been done and observed. The booklet will be countersigned by the department tutor and by the tutor-coordinator. The intern will declare that he has received an ongoing opinion from the tutor regarding the progress of the internship, by signing in the appropriate space of the booklet.

The Department tutor will inform the trainee on the outcome of the attendance itself, especially highlighting any non-positive feedback, so that the trainee can improve in the following period of the month’s salary. The Tutor-coordinator of the medical or surgical area, identified by the Council of the Master’s degree course, after evaluating the intermediate judgments of the other tutors, will be in charge of formulating the final judgment of suitability or unsuitability, communicating the overall judgment to the trainee on the monthly internship carried out. In the event of ineligibility, the trainee will be required to repeat the attendance and to have a new judgment on the same month’s salary.

The tutor-coordinator will keep the booklet containing the student’s diary, the intermediate assessments and the final collegiate assessment; you will take care to send this document to the Administrative Secretariat. The Administrative Secretariat, after having received the three booklets referring to the three areas of the practical evaluation internship, having verified the passing of the three months, will insert them in the student’s file and transfer them to the commission for evaluation in the final exam.

The Council of the master’s degree course in Medicine and Surgery will annually appoint the tutor-coordinators, responsible for the operating unit, who will have the task of certifying the internship in the medical and surgical area. The general practitioners affiliated with the NHS will be identified in accordance with the Medical Association, on the basis of specific conventional documents stipulated between the Medical Association and the University.

The internships that may have TPVES value will be indicated in the annual teaching planning document, without prejudice to the provisions of articles 1 and 4 of the Ministerial Decree of 9 April 2020 n. 12.

The tutor-coordinators and the related medical and surgical area departments are listed in the specific tables attached to these regulations and which will be updated at the beginning of the academic year.

The Medical Area and Surgical Area Departments, as well as the General Practitioners to whom individual Students will be referred, will be indicated directly by the Teaching Secretariat or by another body identified by the University, on the basis of the prior availability of the OUs and Doctors of General Medicine which will in any case be indicated by the Provincial Order of Surgeons and Dentists competent for the area.

TRANSITIONAL RULES

As expressly provided for by the Rector’s Decree n. 1179/2020 of 22 June 2020, the 15 credits used for the purposes of the practical-evaluation internship, for students enrolled from the 2020-2021 academic year, are expressly identified within the scheduled teaching. On a transitional basis, the professional credits used by students enrolled in previous years are present in the integrated courses of the fifth and sixth year of the course. The credits of the teaching activities chosen by the student may also be used, especially in relation to the medical and surgical disciplines present in the fourth year of the course. These credits are attended only once and have a double value; therefore they must not be repeated twice, one for the integrated course and one for the practical evaluation internship for the purpose of the professional qualification exam. It is understood that students who are late or out of course, of any order, who will present the application to carry out the practical evaluation internship having already taken all the exams, agree to repeat the attendance of the activities in the medical and surgical area and possibly of the area of ​​general medicine in excess of credits already attended for the purposes of the related exams of the integrated courses to which they belong.

NOT ENABLING DEGREE OPTION

Students who are enrolled in the master’s degree course in Medicine and Surgery, class LM-41 not qualifying, or belong to the class LS/46 system or previous systems, or belong to the degree class LM-41 270/04 who have opted not to carry out the qualifying internship within the training course, they are entitled to conclude their studies, according to the teaching regulations previously in force, carrying out a professional internship, with the achievement of the academic title only. These students are also entitled to subsequently obtain the qualification to practice the profession of doctor-surgeon, with the achievement of the apprenticeship assessment, prescribed by article 2 of the DM 19 October 2001, n. 445.

For the above students, the University will continue to prepare the diploma of achievement of the academic title only and a separate diploma of the state exam referring to the qualification to practice the profession of surgeon obtained pursuant to the Law Decree of 17 March 2020, no. 18, art. 102, paragraph 2.

Professional skills to be achieved at the end of the TPVES

a)  At the end of the medical and surgical area internships, the student must have achieved the following professional skills, in the two specific areas:

b)  At the end of the internship as a General Practitioner, the student must have achieved the following professional skills:

• Implements best practices in the doctor-patient relationship, knows how to manage reception and structure the consultation (interview, report, information, clarity, acquisition of consent)
• Has the ability to take the history and perform a physical examination in an outpatient and home setting
• Knows and knows how to apply clinical reasoning: is able to identify the reasons for the request for help and the nature and priority of the problem
• He is able to evaluate emergencies and identify the need for hospitalization
• Is able to propose diagnostic hypotheses and to identify the first level diagnostic tests with greater sensitivity and specificity to confirm or not the hypotheses
• Can interpret laboratory tests
• Can interpret diagnostic imaging test reports
• It focuses on the decision-making processes relating to the prescription of correct treatment and on the request for specialist advice
• Is able to carry out control activities on patient adherence to therapy and plan monitoring and follow-up
• Knows the problems of the chronic patient with comorbidities in multi-drug therapy
• Demonstrates knowledge of the organization of the National and Regional Health Service and of the main bureaucratic and prescriptive rules
• He is able to use the computerized medical record and knows the information systems of the National and Regional Health Service
• Can indicate preventive actions, health promotion and correct lifestyles
• Respect the start and end times of the shift, dress appropriately for the role, bring everything necessary with you
• Demonstrates knowledge and awareness of the rules of organization and operation of the medical office
• Interacts correctly with the secretarial and nursing staff of the general practitioner’s office
• Demonstrates an active and collaborative attitude (asks questions, proposes to carry out activities)

Linee guida per il TPVES

Accesso al Tirocinio Pratico Valutativo (TPVES)

Per lo svolgimento del Tirocinio Pratico Valutativo e il conseguimento della laurea abilitante ai sensi del DL. n.18 del 17 Marzo 2020 è necessario assolvere ai seguenti adempimenti:

1. Aver superato il blocco di ordinamento per l’iscrizione al VI anno
2. Compilare la richiesta di svolgimento del TPVES, e trasmettere il PDF firmato alla segreteria didattica inserendolo nel modulo Microsoft Forms della scheda anagrafica.
3. Le assegnazioni ai reparti di area medica e chirurgica saranno effettuati dalla Segreteria Didattica, successivamente alla presentazione del modulo di richiesta e compatibilmente con la tempistica della sessione di Laurea alla quale si intende adire.
4. Le assegnazioni ai Medici di Medicina Generale vengono effettuate dall’Ufficio Esami di Stato generalmente per il mese antecedente la sessione di laurea alla quale si intende adire.

Regolamento del TPVES

Per quanto previsto dalle normi vigenti, il tirocinio pratico-valutativo, finalizzato al conseguimento dell’abilitazione per l’esercizio della professione di Medico-Chirurgo (ai sensi del DL n. 18 del 17/03/2020), da svolgersi nel periodo pre-laurea:

• è volto ad accertare le capacità dello studente relative al saper fare e al saper essere medico, che consiste nell’applicare le conoscenze biomediche e cliniche alla pratica medica, nel risolvere questioni di deontologia professionale e di etica medica, nel dimostrare attitudine a risolvere problemi clinici afferenti alle aree della medicina e della chirurgia e delle relative specialità, della diagnostica di laboratorio e strumentale e della sanità pubblica, ad applicare i principi della comunicazione efficace;
• dura complessivamente tre mesi, è espletato non prima del quinto anno di corso e purché siano stati sostenuti positivamente tutti gli esami fondamentali relativi ai primi quattro anni di corso, previsti dall’ordinamento didattico del Corso di laurea, ed è organizzato secondo quanto stabilito dall’ordinamento e dal regolamento didattico;
• si svolge per un numero di ore corrispondenti ad almeno 5 CFU per ciascuna mensilità (ad ogni CFU riservato al tirocinio debbono corrispondere almeno 20 ore di attività didattica professionalizzante e non oltre 5 ore di studio individuale) e si articola nei seguenti periodi, anche non consecutivi: un mese in area chirurgica, un mese in area medica, un mese nello specifico ambito della Medicina Generale, quest’ultimo da svolgersi non prima del sesto anno di corso, presso l’ambulatorio di un Medico di Medicina Generale. Il tirocinio pratico-valutativo si potrà svolgere anche nei periodi in cui non è programmata attività didattica frontale (generalmente gennaio, febbraio, giugno, luglio, agosto, settembre) per il raggiungimento delle 100 ore certificabili. I 15 CFU riservati alle attività del tirocinio pratico-valutativo fanno parte dei 60 CFU dedicati all’attività didattica professionalizzante;
• la certificazione della frequenza e la valutazione dei periodi di tirocinio avvengono sotto la diretta responsabilità e a cura del docente universitario o del dirigente medico, responsabile della struttura frequentata dal tirocinante, e dal medico di Medicina generale, che rilasciano formale attestazione della frequenza ed esprimono, dopo aver valutato i risultati relativi alle competenze dimostrate, in caso positivo, un giudizio di idoneità, su un libretto-diario, che si articola in una parte descrittiva delle attività svolte e di una parte valutativa delle competenze dimostrate;
• si intende superato solo in caso di conseguimento del giudizio di idoneità per ciascuno dei tre periodi.

NORME SPECIFICHE

Ogni tirocinio di area medica o chirurgica, si potrà svolgere anche in diverse divisioni cliniche di area medica o di area chirurgica, all’interno delle U.O.. In questo caso, ogni tutor delle diverse divisioni rilascerà un giudizio sintetico sul candidato, da cui deriverà il giudizio di valutazione complessiva del candidato, ad opera del tutor coordinatore di area medica e/o chirurgica; Il tirocinio dal medico di medicina generale si svolgerà presso i medici indicati dall’Ordine professionale. Tali medici, individuati dall’Ordine/dagli Ordini dei Medici chirurghi e degli Odontoiatri di pertinenza ed assegnati agli studenti dall’Ufficio esami di Stato, esprimeranno il giudizio di valutazione complessiva del candidato.

NORME ATTUATIVE

La frequenza al tirocinio pratico valutativo ha inizio previa presentazione di apposita richiesta, compilata dallo studente, da consegnarsi alla Segreteria didattica del Corso di laurea magistrale in Medicina e Chirurgia. Lo studente dovrà allegare copia di un valido documento di identità. Allo studente saranno assegnati i reparti di area medica e chirurgica ed il medico di medicina generale dove lo studente dovrà frequentare, sulla base dell’Organizzazione didattica del quinto e sesto anno di corso. La Segreteria Didattica controllerà la veridicità delle dichiarazioni contenute nelle richieste, ai sensi della normativa vigente.

Allo studente verranno consegnati, in sequenza, tre libretti, uno per la frequenza in area medica, uno per la frequenza in area chirurgica, uno per la frequenza presso l’ambulatorio del medico di medicina generale convenzionato con il SSN. Lo svolgimento del tirocinio pratico-valutativo, da parte dello studente dovrà comprendere 100 ore di frequenza; tali periodi di frequenza, compatibilmente con l’Organizzazione didattica del Corso di laurea Magistrale, non dovranno necessariamente coincidere con la durata di un mese; anche la successione tra i diversi periodi, per ogni tirocinante, sarà conseguente all’organizzazione didattica del Corso di Studi e dovrà permettere successioni diversificate nelle diverse aree, in modo da consentire il corretto svolgimento delle attività, fatto salvo che il tirocinio dal medico di medicina generale si potrà svolgere esclusivamente e continuativamente, nel sesto anno di corso.

Sul libretto saranno annotati i giorni e gli orari delle frequenze, descrivendo le attività svolte anche con spunti riflessivi su quanto fatto e osservato. Il libretto sarà controfirmato dal tutor di reparto e dal tutor-coordinatore. Il tirocinante dichiarerà di aver ricevuto dal tutor un parere in itinere relativo all’andamento del tirocinio stesso, firmando nell’apposito spazio del libretto.

Il tutor di Reparto darà informazione al tirocinante sull’esito stesso della frequenza, mettendo in evidenza soprattutto eventuali riscontri non positivi, in modo tale che il tirocinante possa migliorare nel periodo successivo della mensilità. Il Tutor-coordinatore dell’area medica o chirurgica, individuato dal Consiglio del Corso di laurea Magistrale, dopo aver valutato i giudizi intermedi degli altri tutor, si incaricherà di formulare il giudizio finale di idoneità o di non idoneità, comunicando al tirocinante il giudizio complessivo sulla mensilità di tirocinio svolta. In caso di non idoneità il tirocinante sarà tenuto a ripetere la frequenza e ad avere un nuovo giudizio sulla stessa mensilità.

Il tutor-coordinatore tratterrà il libretto contenente il diario dello studente, i giudizi intermedi e il giudizio collegiale finale; avrà cura di far pervenire tale documento alla Segreteria Amministrativa. La Segreteria Amministrativa, dopo aver ricevuto i tre libretti riferiti alle tre aree del tirocinio pratico valutativo, verificato il superamento delle tre mensilità, li inserirà nel fascicolo dello studente e li trasferirà alla commissione per la valutazione nella prova finale.

Il Consiglio del Corso di laurea magistrale in Medicina e Chirurgia nominerà annualmente i tutor-coordinatori, responsabili di unità operativa, che avranno il compito di certificare il tirocinio in area medica e chirurgica. I medici di Medicina generale convenzionati con il SSN saranno individuati in accordo con l’Ordine dei Medici, sulla base di specifici atti convenzionali stipulati tra Ordine dei Medici ed Ateneo.

Nel documento di programmazione didattica annuale saranno indicati i tirocini che potranno avere valenza TPVES, fatto salvo quanto previsto agli articoli 1 e 4 del DM del 9 Aprile 2020 n. 12.

I tutor-coordinatori ed i relativi reparti di area medica e chirurgica sono elencati nelle specifiche tabelle allegate al presente regolamento e che saranno aggiornate all’inizio dell’anno accademico.

I Reparti di Area Medica e di Area Chirurgica, così come i Medici di Medicina Generale cui saranno indirizzati i singoli Studenti, saranno indicati direttamente dalla Segreteria Didattica o da altro organo individuato dall’Ateneo, sulla base delle preventive disponibilità delle U.O. e dei Medici di Medicina Generale che saranno comunque indicati dall’Ordine Provinciale dei Medici Chirurghi e degli Odontoiatri competenti per territorio.

NORME TRANSITORIE

Per quanto espressamente previsto dal Decreto Rettorale n. 1179/2020 del 22 giugno 2020, i 15 CFU utilizzati ai fini del tirocinio pratico-valutativo, per gli studenti immatricolati dall’anno accademico 2020-2021, sono identificati espressamente all’interno della didattica programmata. In via transitoria, i CFU professionalizzanti utilizzati dagli Studenti immatricolati negli anni precedenti, sono presenti nei corsi integrati del quinto e sesto anno di corso. Potranno altresì essere utilizzati i CFU delle attività didattiche a scelta dello studente, soprattutto in relazione alle discipline mediche e chirurgiche presenti nel quarto anno di corso. Questi CFU sono frequentati una sola volta e sono a doppia valenza; non debbono pertanto essere ripetuti due volte, una per il corso integrato e una per il tirocinio pratico valutativo ai fini dell’esame di abilitazione professionale. È inteso che gli Studenti in ritardo o fuori corso, di qualsiasi ordinamento, che presenteranno la domanda per svolgere il tirocinio pratico valutativo avendo già sostenuto tutti gli esami di profitto, accettano di ripetere la frequenza delle attività in area medica e chirurgica ed eventualmente dell’area di medicina generale in sovrannumero rispetto a CFU già frequentati ai fini dei relativi esami dei corsi integrati cui appartengono.

OPZIONE DI LAUREA NON ABILITANTE

Gli studenti che risultano iscritti al Corso di laurea magistrale in Medicina e Chirurgia, classe LM-41 non abilitante, o appartengono all’ordinamento della classe LS/46 o ordinamenti previgenti, o appartenenti alla classe di laurea LM-41 270/04 che abbiano optato per non svolgere il tirocinio abilitante all’interno del percorso formativo, hanno titolo a concludere gli studi, secondo l’ordinamento didattico previgente svolgendo un tirocinio professionalizzante, con il conseguimento del solo titolo accademico. Detti studenti hanno, altresì, titolo a conseguire successivamente l’abilitazione all’esercizio della professione di medico-chirurgo, con il conseguimento della valutazione del tirocinio, prescritta dall’articolo 2 del D.M. 19 ottobre 2001, n. 445.

Per gli studenti di cui sopra, L’Ateneo continuerà a predisporre il diploma di conseguimento del solo titolo accademico e un separato diploma di esame di Stato riferito alla abilitazione all’esercizio della professione di medico chirurgo conseguita ai sensi del Decreto Legge 17 marzo 2020, n. 18, art. 102, comma 2.

Competenze professionali da raggiungere al termine del TPVES

a) Al termine dei tirocini di area medica e chirurgica, lo studente dovrà aver raggiunto le seguenti competenze professionali, nelle due aree specifiche:

b) Al termine del tirocinio dal Medico di Medicina Generale, lo studente dovrà aver raggiunto le seguenti competenze professionali:

• Mette in atto le buone pratiche del rapporto medico-paziente, sa gestire l’accoglienza e strutturare la consultazione (colloquio, relazione, informazione, chiarezza, acquisizione del consenso)
• Ha la capacità di raccogliere l’anamnesi e di eseguire un esame obiettivo in un contesto ambulatoriale e domiciliare
• Conosce e sa applicare il ragionamento clinico: è in grado di individuare i motivi della richiesta di aiuto e la natura e priorità del problema
• È in grado di valutare le urgenze ed individuare le necessità per un ricovero ospedaliero
• È in grado di proporre ipotesi diagnostiche e di individuare gli accertamenti diagnostici di primo livello dotati di maggiore sensibilità e specificità per confermare o meno le ipotesi
• È in grado di interpretare gli esami di laboratorio
• È in grado di interpretare i referti degli esami di diagnostica per immagini
• Si orienta sui processi decisionali relativi alla prescrizione di un corretto trattamento e sulla richiesta di una consulenza specialistica
• È in grado di saper svolgere attività di controllo sull’adesione alla terapia da parte del paziente e programmare il monitoraggio e il follow up
• Conosce le problematiche del paziente cronico con comorbidità in terapia plurifarmacologica
• Dimostra conoscenza circa l’organizzazione del Servizio Sanitario Nazionale e Regionale e sulle principali norme burocratiche e prescrittive
• È in grado di utilizzare la cartella clinica informatizzata e conosce i sistemi informativi del Servizio Sanitario Nazionale e Regionale
• Sa indicare azioni di prevenzione, di promozione della salute e corretti stili di vita
• Rispetta gli orari di inizio e fine turno, veste in maniera adeguata al ruolo, porta con sé tutto il necessario
• Dimostra conoscenza e consapevolezza delle regole di organizzazione e funzionamento dello studio medico
• Interagisce correttamente col personale di segreteria ed infermieristico dello studio del medico di medicina generale
• Dimostra un atteggiamento attivo e collaborativo (fa domande, si propone per svolgere attività)
